Pennsylvania

Finding the right care for an aging parent or loved one can be overwhelming.

Pennsylvania families have access to a wide range of senior care services, from in-home assistance and community programs to assisted living, memory care, rehabilitation, skilled nursing, and hospice.

But knowing which type of care is right can be difficult.

CareFlow Partners helps families understand their options, explore local resources, and take the next step toward a care plan that fits their loved one's needs.

Whether you're planning ahead or facing a sudden change in your loved one's health, you don't have to figure everything out at once.

Start by understanding the situation. Then explore the options.

Understanding Senior Care in Pennsylvania

Senior care isn't one-size-fits-all.

An older adult who needs help with meals and transportation may be able to remain safely at home.

Someone who needs assistance with bathing, dressing, medications, and daily activities may benefit from personal care or assisted living.

Someone experiencing significant cognitive decline may require specialized memory care.

And someone recovering from surgery, a serious illness, or a hospitalization may need short-term rehabilitation or skilled nursing.

The goal isn't to automatically choose the highest level of care.

The goal is to find the most appropriate and safe level of support for the individual.

Pennsylvania Aging Resources

Pennsylvania has an extensive network of organizations designed to help older adults remain independent and access appropriate services.

The Pennsylvania Department of Aging oversees statewide programs and works with Pennsylvania's network of local Area Agencies on Aging.

Local Area Agencies on Aging can help families connect with:

  • Meals

  • Transportation

  • Care management

  • Caregiver support

  • In-home services

  • Senior centers

  • Benefits counseling

  • Community programs

  • Long-term care resources

For many families, the local Area Agency on Aging can be an excellent place to start when they don't know where to turn.

CareFlow Partners in Pennsylvania

CareFlow Partners is building a growing network of local senior-care resources throughout Pennsylvania.

Our goal isn't simply to provide a directory of facilities.

We want to help families understand:

What is happening?

What type of care might be appropriate?

What local resources are available?

How much might care cost?

What should we do next?

Our Care Navigator is designed to help families organize these questions and identify care options worth exploring.
